Locum Consultant in Intensive Care

The Department
The ICU department consists of 23 beds, these beds are split into a separate 9 bedded unit which houses 7 level-3 beds and 8 level-2 beds. This managed flexibly by altering the beds depending on the patient requirements.
Within the department there are currently 10 consultants who work dedicated sessions in critical care and providing a separate on-call rota, this is established in a ‘block’ pattern to ensure the continuity of care for the patients. The majority of the ICU Consultant Intensivists have Anaesthetics sessions that fall into their non-ICU weeks based on a 10-week rolling job plan which is scheduled around their primary ICU duties. This however is not required if you are applying as a medicine trained Intensivist.
The ICU unit see’s approximately 600 patients a year with about 60% of them being medical and the reminding 40% being split between elective and emergency surgical patients. The surgical specialists which are covered by this department on site are colorectal, vascular, urology, ENT, T&O, gynaecology and obstetrics.
The department is equipped to provide and perform ultrasonography, echocardiography, cardiac output monitoring and all standard critical care such as invasive and non-invasive lines as well as renal replacement therapy and high nasal oxygen therapy for up to eight patients.
Main Duties of the job
Your duties will be primarily based at one site however your presence may from time to time be required in other parts of the Trust. The key duties and responsibilities of the post will be:
- Participate in the dedicated ICU on-call rota with other colleagues
- Provide day time cover for Critical Care and support the Outreach service
- Take an active part in the supervision and management of junior medical staff, including formal teaching programmes and clinical audits
- Participate in appraisal and assessment of junior medical staff and undergraduates, and performance of clinical audit as delegated by the Unit Training Director
- To teach and train staff in other medical disciplines, nursing and support staff as appropriate
- Work with colleagues to provide a Consultant-led service with the highest standard of care
- Work with the multi-professional team to develop care pathways and clinical guidelines
- Develop and maintain good working practices
- Collaborate and promote close working links with colleagues, other departments and primary care
- Offer educational, clinical support and leadership to junior medical colleagues, nursing colleagues and other members of the multi-disciplinary team
- Ensure appropriate information is made available to staff to measure clinical performance and enable meaningful benchmarks to be established and evaluated within their area of responsibility
- Ensure there is a robust process for reviewing the quality of services provided, explaining the causes of success and failure and ensure the highest standards are maintained
- Provide a role model of professional leadership, which underpins the corporate values and behaviour agreed by the organisation, ensuring these are understood and demonstrated by the medical workforce within their area and take remedial action where poor behaviours are demonstrated
- Ensure that junior doctor’s training conforms to College requirements
- Participate in the appraisal of medical staff within the department in line with the Trust’s agreed policy
Opportunities for development
Approval has been given for an exciting new state-of-the-art elective orthopaedic centre (EOC) and they are at the preliminary stages of planning and design. This will be a £44m build and will house approximately 50 beds and 6 operating theatres. All elective orthopaedic work from the region will be performed here. It is predicted to be opened in 2023. The significant anaesthetic involvement in the layout of the new theatres and the day surgery area will also ensure that separate areas are allocated for regional anaesthetics and nerve blocks.

The Department
The Intensive Therapy Unit (ITU) is a 9-bed area with a mixture of both intensive and high dependency level patients from medicine, surgery, and obstetrics. They admit approximately 480 patients a year and deal with many more at ward level via the Health Board's outreach service.
There are six intensive therapy Consultants, an anaesthetics core trainee undergoing their ITU rotation, ACCS Emergency Medicine trainees, an FICM trainee and an F1/F2 doctor. They attend all cardiac arrest / trauma calls, including emergencies in obstetrics and paediatrics. The main hospital site does not have a paediatric intensive care but, in conjunction with the anaesthesia team, they do provide the initial resuscitation and stabilization of the sick child before they are ultimately repatriated by a specialist team to a tertiary unit.
Working in the ITU you would see the sickest patients in the hospital, from emergency patients of all ages via the emergency department and wards, to major post-operative cases. You would also be part of a multi-disciplinary team where the day-to-day business will include the assessment and admission of patients requiring advanced airway (including tracheostomy) and vascular access skills, ward rounds, arranging and preparing patients for intra-hospital investigations and liaising with specialist teams. The senior team will ensure that you are supported to develop your clinical skills and career aspirations throughout your time with them. This may include exposure to organ donation, focused echocardiography, advanced research, further education opportunities and wellbeing development sessions.
Main Duties
- You will be expected to deputise for absent colleagues and in exceptional unforeseen circumstances may have to perform additional duties to ensure the safe running of the department. Where such duties take place outside your normal contracted duties, you will receive either an equivalent off duty period or will be remunerated appropriately.
- You will be required to participate in the Health Board’s (HB) risk management process and in clinical audit and other clinical governance activities within the department, the directorate, and the HB. Adherence to Caldicot Principles is mandatory.
- You will be required to participate in the HB’s Appraisal Scheme in conjunction with your Supervising Consultant, to identify training and development needs. You will be expected to participate in personal and professional development to fulfil Royal College CME requirements.

Teaching
You will be encouraged to become involved with local departmental teaching; this involves the development and implementation of simulator-based teaching for Anaesthetic and ICM trainees in one of the 3G simulation suites supported by teaching mannikins and a comprehensive library of resources. You will also be involved in running a regular journal club, exam practice and critical incident scenarios.
The permanent members of staff have a range of expertise and many years of experience, along with commitments within the school of Anaesthesia, Royal College of Anaesthetists, Faculty of Intensive Care Medicine and Cardiff University. Specialist Cardiothoracic Anaesthesia

The Department
With up to 1000 cardiac surgical cases per year and a very busy thoracic and vascular surgical programme, this is an opportunity to work within one of London’s best teaching centres for cardiothoracic anaesthesia training.
Complex and high-risk surgery is undertaken daily within the departments dedicated operating theatres, providing intensive training opportunities for speciality registrars and specialists. The team runs a busy programme of challenging cardiology catheter lab procedures including electrophysiology and transcutaneous valve replacement (TAVI).
The 21 bedded intensive care unit admits elective and emergency cardiac surgical patients, vascular surgical patients, and an increasing proportion of acute cardiology patients – including primary PCI and out of hospital arrests.
As a Specialist within the department your work and progression will be supported by the teams 10 specialist registered Cardiothoracic Anaesthetic Consultants.
Main Duties of the job
- The job rota will initially be based on a 10 PA (40 hour week)
- You will be covering the 1 in 7 on-call rota
- Preoperative evaluation and postoperative care on the ward as well as within the cardiac intensive care unit.

The Department
The Critical Care Unit is situated on the First Floor; 9 bedded facility including 6 level III beds and 3 level II (surgical HDU) beds, though beds are used flexibly. It is a modern facility with state-of-the-art monitoring and capacity for cardiac output monitoring (LidCO and ODM), invasive monitoring and haemofilteration (using Prismaflux machines). The unit has recently purchased a high frequency oscillator for use in severe hypoxaemia.
The ICU unit comprises of a 9 bed Intensive Care Unit. With regards to the national challenge of the COVID 19 situation, there is business continuity to provide additional capacity should the need to arise in the future.
The theatre suite consists of nine theatres, a major treatment room within the single site and one maternity theatre, all of which are equipped to modern standards. There are dedicated paediatric and adult recovery areas and facilities to manage high care patients for resuscitation. There is a separate Obstetric Suite with operating Theatre.
Main Duties of the job
The unit is covered weekdays by a consultant with an interest in ITU (forms part of a 9-week rota) and two staff grade or trainee. One of these two staff grades or trainees is responsible to take outreach referrals under the supervision of Consultant. Trainees rotate on occasion through ITU, which is accredited for basic and intermediate ICM training. There is ICM consultant ward rounds twice daily as required by ICS standards and RCoA. There is a separate Critical Care and General Consultant on call rota.
The hospital benefits from a team of band 7 Outreach nurses who have a close working relationship with ITU and Theatres. The unit admits over 300 patients per year, two thirds of which receive ventilator and multi organ support. It participates in the ICNARC and other initiatives through the GM Critical Care Network.
Once a month clinical work is cancelled for the majority of one day to protect time for audits.

Locum Consultant in Anaesthetics with an interest in Obstetric Anaesthesia

On-calls will involve covering theatres in surgical, trauma, obstetric and paediatric emergencies. Within your job plan you will have a definitive 37.5 hours per week, the equivalent to 10 sessions of work. If you are successful during interview, you will have the opportunity to arrange SPA’s within this dedicated to CPD. The Anaesthetic department is also extremely supportive of individuals seeking employment with the Emergency Medical Retrieval and Transfer Service or Adult Critical Care Transfer Service as part of their weekly job plan.

The Department
The department covers two vibrant NHS district general hospitals which provide acute surgical, medical and elective surgery services to the area. The team provides all consultant led obstetric care and paediatric services for the south of Wales and all ENT surgery for the Health Board. Locum Consultant in Vascular Anaesthesia
This is a new post for a Consultant Anaesthetist with an interest in Anaesthesia for major General and Vascular Surgery. Due to the expansion of the vascular service in particular, the job is suitable for those who would like to develop their vascular skills and robotic surgery exposure. Interests of successful candidates can be taken into consideration following appointment.
The job plan consists of 10 PA’s per week including core SPAs of 1.5 The timetable will usually include fixed or flexible DCC PAs for operating theatre sessions, pre-assessment and attendance at MDT as required. The out-of-hours rota frequency is currently 1 in 13. You will also be allocated 0.5PA per week for peri-operative care, general administrative duties, notes reviews and other related clinical duties.

The Department
The department is made up of a friendly team of 31 Consultant Anaesthetists, including nine Critical Care Consultants and three chronic pain Consultants. The wider team also boasts consists of Associate Specialists, a team of Specialty Doctors and trainees from the Foundation Programme and Eastern Region School of Anaesthesia.
The department boasts 17 operating theatres across three suites, this included a 24 hour dedicated emergency theatre. The Anaesthetics department has a dedicated hybrid theatre for vascular and interventional radiological procedures situated adjacent to recovery and the Critical Care Unit. This theatre has a large anaesthetic room to facilitate line insertion and regional anaesthesia, a built-in C-arm and mobile lead screens for radiation protection and a ‘Haemochron’ ACT machine.
The Vascular Services provided at the Trust are tertiary, covering all complex major vascular procedures including non-fenestrated and fenestrated endovascular and open aortic aneurysm repairs, carotid endarterectomy and distal revascularisations. This service is supported by weekly Consultant-led vascular preassessment clinics seeing all major vascular cases, Consultant Anaesthetist attendance at vascular MDT and cardiopulmonary exercise testing for all patients requiring aortic surgery for non-occlusive disease
Main duties of the job
- Following your Trust and departmental induction, you will be expected to participate in the on call consultant rota, covering general emergencies, vascular emergencies and obstetrics.

- ECFMG verification
- Subspecialty advanced training within Vascular Anaesthesia
- Experience working in a similar role at Consultant grade within the NHS or Overseas
- Solid understanding of the CCT or CESR process
